Office of the College Registrar

The Office is responsible for efficient management and maintenance of all pertinent student records such as personal student information, grades and other pertinent data; and the effective and efficient delivery of service where student records are concerned.

The Office maintains the academic records of the students from enrollment  to graduation from their chosen degree program. All documents submitted and transactions pertaining to student academic progress are accurately  filed at the student’s folders.


Functions/ Services Offered

  1. Maintenance of the permanent student academic records of all its current and former students and graduates;
  2. Issuance of official academic records such as official transcript of records, diploma, and certifications like:
    1. Certificate of Grades for scholarship purposes or whatever purpose it may serve.
    2. Certificate of Enrollment for scholarship purposes or whatever purpose it may serve.
    3. Certificate of General Weighted Average for whatever purpose it may serve.
    4. Certificate of Earned Units for whatever purpose it may serve.
    5. Certificate of English as Medium of Instruction for Employment purposes.
    6. Certificate of Earned Units for Employment purposes.
    7. Certificate of Underload Units for CHED Scholarship ( UNIFAST/ TDP scholarship )
    8. Certification Authentication and Verification of documents ( CAV/ Red Ribbon to DFA)Students who wish to apply for CAV to the Department of Foreign Affairs will:
      1. Get and accomplish Request for document form ( from 2) at theRegistrar’s  Office;
      2. Submit a photocopy of Diploma and Official Transcript of Record
        which will be authenticated by the Registrar;
      3. Pay corresponding fee to the Accounting office;
      4. Submit the accomplished form at the Registrar’s office;
      5. The office  will prepare the endorsement documents which will be submitted to CHED-CAR.
    9. Other certifications as requested by students like Certificate of Bonafide student.
    10. Authentication of academic documents.
      Students may have their record/document authenticated .
      1. The student is required to pay the corresponding fee for authentication of record at the accounting office.
      2. The staff In-charge of requests shall countercheck the authenticity of the record/document to be authenticated.
      3. The remark, CERTIFIED TRUE COPY , shall be marked and signed by the Registrar.
    11. Responsible for the verification of academic information of all its students and graduates as requested by various government or private agencies and institutions.

Procedure and Policy  for Requesting

  1. Get and accomplish Request form ( form 2 ) from the Registrar’s Office. Check or write the type of request.
  2. Pay at the accounting office.
  3. Submit the accomplished request form at the Registrar’s office.
  4. The office will issue a claim slip.
  5. Claim after 3 working days if Certificates and ten working days if OTR and Diploma from the date of submission of the request.
  6. If unclaimed within sixty ( 60 ) days from the date of application shall be disposed.

NOTE: Saturday and Sunday is not counted as a working day for determining due date of request.

The Office of the College Registrar delivers outstanding services and support to our customers with the highest standard of integrity, accuracy, efficiency, confidentiality and security.  

Objectives

  1. Administer an efficient registration and scheduling process to deliver the College’s curriculum in accordance with its policies and regulations.
  2. Provide timely, precise guidance and support to students as they work toward degree completion, as well as faculty and staff involvement in the graduation process.
  3. Ensure that the College maintains accurate personal, academic and enrollment records for its entire student population, past and present, and provide access to data derived from these records only when appropriate.
  4. Demonstrate a philosophy of proactive leadership, collaboration and continual assessment that improves outcomes within the office and throughout the College.
